Passports

The Town Clerk's Office is a Passport Acceptance Facility.  Passport photo services are available for a fee of $12.00.  We accept applications until 3:45pm, Monday-FridayAppointments for passport processing are required.  Call our office during normal business hours to schedule. Passport Forms are available in our office or they can be downloaded.

It is currently taking 8-12 weeks to receive your passport.  If you need your passport sooner, it is recommended to use expedited service (up to 7 weeks).  If you need it within 14 days, call
1-877-487-2778 to make an appointment at a Regional Passport Agency.

Personal check or money order is needed for each passport application.
HOW TO APPLY FOR THE FIRST TIMEEach person applying must appear in person.

  1. A COMPLETED APPLICATION (DS-11)
  2. PROOF of U.S. CITIZENSHIP (original must be mailed with the application and will be returned)
    1. Previous U.S. Passport
    2. Certified birth certificate** - must include your full name, the full name of your parent(s), date and place of birth, sex, date the birth record was filed, and the seal or other certification of the official custodian of such records
    3. Certificate of Naturalization
    4. Certificate of Citizenship
    5. U.S. Consular Report of Birth
  3. PROOF of IDENTITY - must contain your signature AND physical description or photograph that is a good likeness of you
    1. Previous or current U.S. passport book or passport card
    2. Driver's License
    3. Certificate of Naturalization
    4. Certificate of Citizenship
    5. Military Identification
    6. Federal, State or municipal government employee identification card or pass
    7. temporary or altered documents are not acceptable
  4. PROVIDE a PHOTOCOPY of the ID document that is presented with the application (both front and back on the same side of one sheet)
    1. TWO RECENT COLOR PASSPORT PHOTOS
    2. Identical, of you alone and taken within the last six months and 2X2 inches in size
    3. photograph must be color, clear, with full front of your face, and printed on thin paper with a plain light (white or off-white) background

      • Applicants 16 and older - $165.00
        • one check for $130.00 payable to the U.S. Dept. of State
        • one check for $35.00 payable to the Town of Ogden
      • Applicants under 16 - $135.00
        • one check for $100.00 payable to the U.S. Dept. of State
        • one check for $35.00 payable to the Town of Ogden
    2. Passport Card
      • Applicants 16 and older - $65.00
        • one check for $30.00 payable to the U.S. Dept. of State
        • one check for $35.00 payable to the Town of Ogden
      • Applicants under 16 - $50.00
        • one check for $15.00 payable to the U.S. Dept. of State

Special Requirements for children under 16In addition to the steps above, each child under age 16 must appear with both parents or the child's legal guardian(s) and present the following:

  • Evidence of the child's relationship to parents/guardians(s), AND
  • Parental/guardian identification
Special Requirements for minors age 16 and 17In addition to the steps above, each minor age 16 and 17 must:

  • Have a parent or guardian present photo identification if minor does not have identification of his or her own
  • Establish parental consent (you may be requested to provide written parental consent for a child age 16 or 17 if it is not applied on the application)
